Your employee number will consist of the last 5 digits of your social security number followed by the 2-digit month, day, and year of your date of birth. 

For example: If you were born on January 1, 1970 and your social security number is 123-45-6789 your employee number would be: 56789010170

Please enter your Date of Birth in the format mm/dd/yyyy (This format with slashes is required)
